Regarding the 2 point conversion in the NFL, is a team awarded one or two points if they line up for a kick.?

If, after scoring a touchdown, an NFL team lines up to kick for the extra point and the ball is fumbled by the place kick holder and he (the holder) picks up the ball and runs it across the goal line (or successfully pes it to an eligible receiver in the end zone), is the team awarded 1 or 2 points. Also, what is the ruling on awarding of points for a blocked kick being advanced into the end zone?
